Create Task - Independent Reading Challenge

This is an image of my google form I created for my 'create task' for my independent reading challenge. It includes questions about the story 'A Small Footprint - A Big Impression'. Some of the questions are text, and others are tick box's . Students complete this form once they have read the article.
